Carrot circles with thin strips of green onion leaves would make orange ballons. Or grape tomatoes for red ones. You could also use round slices of radishes for red and white balloons. Single noodles could make strings.

A thin strip of vegetable with pickled daikon (or a red or yellow pepper) 'flame' makes a candle.

You could make little presents out of rice and use vegetable strips/nori to make the ribbons/bows! (and you could leave the rice plain or food color it!) for a cake I'd just make it out of rice I think, and use vegetable strips or food-colored cheese maybe? as decoration (I was thinking of a white cake with blue stripes or something since that seems to be a common color scheme :) :) ) good luck, I hope your bf likes it!

You could also pick up some wax paper or parchement paper, and wrap each container of the bento (assuming you use one with containers with paper, and tie it closed with red string. Each section has to be unwrapped as its own birthday gift. Also, make some cookies or something punched out with his age.

You could make something as simple as a sandwich look like a cake by cutting it into circles with a large cookie cutter - Cut thinly sliced cheese or meat to hang over it in 'drips' for frosting, and make a triple-layer 'cake'! :) You could use small carrot sticks for 'candles'. Also, inari zushi make cute packages :)

Like Fusionbento I made a mini meatloaf in a foil cupcake holder and put food colored mashed potatoes on top as icing for april fools day once. I also used slices of pound cake (toasted) with orange icing sandwiched between them for grilled cheese. The joke was that I told my kids they could eat dessert first. They were thrilled untill they realized the cupcake wasn't dessert after all.
